What Is Cagrilintide + Semaglutide?
This is a co-lyophilized blend of cagrilintide (5 mg) and semaglutide (5 mg) in a single 10 mg vial. The two compounds act through unrelated receptor families: cagrilintide is a long-acting acylated analogue of human amylin that agonizes amylin and calcitonin receptors, while semaglutide is an acylated GLP-1 analogue.
Cagrilintide exists because native amylin is difficult to work with — it forms amyloid fibrils quickly. The published design substitutes prolines taken from rat amylin to suppress β-sheet formation, stabilizes the central helix with a salt bridge, and attaches a C20 fatty diacid at the N-terminal lysine for reversible albumin binding.
Supplied at ≥99% HPLC purity per component with a batch-specific third-party COA reporting both retention times. The first production batch is in final QC; use the form below to request the full analytical package.

| Sequence (1-letter) | 37-residue acylated amylin analogue — see sequence noteNo public one-letter string is reliably published. Kruse et al. (J Med Chem 2021;64:11183-94, compound 23) describe cagrilintide as human amylin carrying 14E, 17R, 25P, 28P and 29P substitutions plus a C-terminal proline change, with an N-terminal C20 fatty-acid conjugate. Vendors also disagree on whether the disulfide is numbered Cys2-Cys7 or Cys3-Cys8. Confirm against your batch COA. |

Two Different Hormone Pathways
This blend pairs an amylin-receptor agonist with a GLP-1-receptor agonist. Amylin is co-secreted with insulin by pancreatic β-cells and signals through amylin and calcitonin receptors; GLP-1 is an incretin released from the gut. The two act on separate receptor families, which is the stated rationale for studying them together.
Cagrilintide's Design
Native human amylin aggregates readily into β-sheet fibrils, which makes it unsuitable as a long-acting agent. Kruse and colleagues addressed this with proline substitutions borrowed from rat amylin, a salt-bridge-stabilized central helix and an N-terminal C20 fatty-acid conjugate for albumin binding.
Receptor Non-Selectivity
Cagrilintide is described as a non-selective agonist at both amylin and calcitonin receptors. Calcitonin-receptor engagement is part of its reported pharmacology rather than an off-target effect, and it is one reason the compound is studied separately from pure amylin analogues.
Co-Lyophilization Considerations
Both components are acylated peptides with similar solubility behaviour, which is why they can share a vial. Reconstituted solutions should be treated as a mixture: the two have different retention times under RP-HPLC, so a batch COA reports purity and content per component rather than a single figure.
Limitations
Cagrilintide's exact published sequence is not publicly settled, and vendor data sheets conflict on the free-base versus acetate-salt formula and mass. Verify identity, content and the salt form against your batch certificate before any analytical work.

Bacteriostatic Water Ratio Chart (10 mg Vial)
| Diluent Added | Concentration | Per 0.1 mL Aliquot |
|---|---|---|
| 1.0 mL | 10.0 mg/mL | 1 mg |
| 2.0 mL | 5.0 mg/mL | 500 µg |
| 2.5 mL | 4.0 mg/mL | 400 µg |
| 3.0 mL | 3.33 mg/mL | 333 µg |
| 5.0 mL | 2.0 mg/mL | 200 µg |
Concentrations shown are total peptide. This vial contains 5 mg Cagrilintide + 5 mg Semaglutide, so each component is a fixed fraction of every aliquot — they cannot be dosed independently.
Reconstitution Procedure
Allow the vial to reach room temperature. Introduce sterile or bacteriostatic water slowly down the inner glass wall — do not inject directly onto the powder cake. Swirl gently until fully dissolved; do not shake or vortex, as this can promote aggregation.

| Lyophilized | 2–8 °C | Up to 12 months |
| Lyophilized | Room temp (≤25 °C) | Several weeks (transit) |
| Reconstituted | 2–8 °C | Up to 28 days |
